If you wish to REGISTER WITH THE PRACTICE please ask at reception. You will be given a short new patient form to complete and offered an appointment with a Health Care Assistant. These appointments are available at different times on different days of the week to make it easier for you to register at a time to suit you. If these times are not convenient please ask to speak to Nicola to make a special appointment for you. Please bring any medication you are on and a urine sample to the appointment. It is essential that you complete the details of your last GP so that we can get your old medical records.

We do not discriminate against anyone wishing to register on the ground of race, gender, social class, religion, sexual orientation, appearance or medical condition. However for technical reasons we are unable to treat persons with USA or Canadian passports. If you fall into this category please speak to one of the doctors.
